Our program is run by licensed nurses who are experienced and knowledgeable in multiple areas
surrounding disease and health conditions. Their responsibilities will include the following:

 

• Blood pressure checks-monitoring for elevated blood pressure    

• Blood glucose checks-monitoring for elevated blood sugar levels

• Height and Weight checks-measuring body mass index (BMI)     

• HIV testing                                                                                     

The nurses will provide education on the risk associated with health conditions such as diabetes,
hypertension, and heart disease through printed handouts and literature. We will also provide
information on ways to prevent these health problems from occurring.

Our Health & Wellness services will be provided to churches, nursing homes, schools, and special
programs per request. Nurses will arrive at locations to provide services upon complete registration.

Please note that ALL services offered by our program are free of charge. Individuals can make a
donation but they are not pressured to do so.

Those participating in the Health & Wellness program rights are protected under the HIPPA act. What
this means is that HRFM will NEVER share personal information including name, phone number, or
